I acknowledge that I am receiving esthetic services at Ariel Esthetic.

I understand and agree to the following:

I have provided accurate information for my personal and contact details.

I have disclosed any allergies, sensitivities, medical conditions, or medications that may affect the services or products used during my appointment.

I have been informed of the potential risks and benefits associated with the chosen service(s), and I am proceeding with the services voluntarily.

I acknowledge that individual results may vary and that specific outcomes are not guaranteed.

I have received and understood any pre- and post-service instructions provided by the esthetician

I give permission to the esthetician to perform the selected services.

I release the esthetician and the studio from any liability related to adverse reactions, injuries, or dissatisfaction resulting from the services provided.

I understand that any discomfort or adverse reactions should be reported to the esthetician immediately.

I consent to before-and-after photos being taken for documentation and marketing purposes (if applicable).

I have read and understood the above information and consent to the services being performed.

NO-SHOW/LATE/RESCHEDULE POLICY

I understand, if I do not show up for my appointment, within a 10-minute grace period, I will need to reschedule and I will forfeit my payment. I understand if I do not cancel my appointment 48 hours, or more, before my scheduled time, I will forfeit my payment. Appointment changes can be made via text, email, or on app. NO EXCEPTIONS please.
